FIRST VISIT
Your first visit to the orthodontic office of Dr. Shaw will be pleasant and informative. You will need to come a few minutes early to fill out a patient history form, so we may get to know you and your concerns. To expedite treatment, you may download our patient form below, fill it out and bring it with you when you visit our office.
After a brief meeting with our treatment coordinator, Dr. Shaw will examine your teeth to determine your orthodontic needs. They will discuss any proposed treatment and give an estimate of length of care and cost. There is no charge for this initial examination, nor is there any obligation to commence treatment.
